Your paperwork can be filled out ahead of time online. This provides us with general information about yourself, your health history and your condition. We do ask if you have had any X-rays or MRI's that pertain to your condition to let us know so we can get them sent over to our office and looked at. We will perform a complete chiropractic examination.

In order to determine what your substantial problem is, we will ask you various questions related to your condition.
